Patient Information
  • Office hours are by appointment only.
  • Office hours are:
    • Kingston Office:Mon. 8am-5pm, Tues. 8:30am-5pm, Wed. 9am-5pm, Thur. 9am-5pm, Fri. 8am-4pm and Sat. 9am-1pm 
    • Catskill Office: Mon. 8am-5pm, Tues. 9am-5pm, Wed. 9am-5pm, Thur. 8am-5pm and Fri. 9am to 4pm. 
    • Office phones are available from 8:30am-4:45pm (Kingston) and 9:00-4:45 (Catskill). The office is open through lunchtime.
  • Each patient chooses a specific office site and a primary care doctor in our group. A History and Physical should be scheduled to establish yourself with that provider.
  • Some of our providers are open to new patients while others only see established patients.
  • After hours, one of the doctors in the group is on call, via the answering service. Please be specific with your complaints and keep your phone open so the doctor can return your call.
  • Annual physical exams should be scheduled 4-6 weeks in advance.
  • Follow up visits should be scheduled at time of visit as indicated by your doctor.
  • Please request prescription refills at least one week in advance.
  • Have your records transferred from your old doctor to our office. This needs to be done in writing. We have the necessary forms. If you belong to an HMO insurance you need to contact them and change your primary care doctor.
  • If you belong to an HMO insurance company, remember that you need a referral before you go to the specialist. There can be no back dated referrals. Please request your referral 1-2 weeks before your scheduled appointment.  Referrals must be picked up at the office.
  • It is important to check in with the receptionist when you first arrive and tell us of any changes in your address, phone or insurance. Please bring your insurance card with you for each visit.
  • Payment is expected at time of visit. THIS INCLUDES ALL CO-PAYS. We accept checks and credit cards (Visa/Master Card/American Express). There is a returned check fee of $20.00.
  • We work at Benedictine and Kingston Hospitals.
  • We generally are able to accommodate most patient acute problems in the office.
  • If you are having a life-threatening” emergency you can go directly to the hospital emergency room. Don’t waste time coming to the office first.
  • Please remember that missed appointments waste the doctors’ time and prevent other patients from being seen. If you must cancel an appointment call the office within 24 hours or you will be charged $25.00 for an office visit or $50.00 for a physical.
